An inclusive step-down placement pathway.
Upon referral to our service, our clinical specialist team will undertake an extensive risk assessment of each young person to ensure their complex specific needs are robustly identified and the appropriate level of therapeutic care is implemented in order to meet such needs.
Step-down Placement Pathway
Oakfield’s ethos is strongly goal-directed, working in partnership with each person to promote and establish independance, resilience and social inclusivity.
Through our recognised targeted model of care, we aim to step down the care each young person receives, ultimately leading to the effective transition into independent or semi-independent living.

- Formation of person-centred care package and collaborative goal setting
- Formation of meaningful relationships and secure attachments
- Development of social interaction and inclusion
- Building of identity and self
- Intensive Trauma focused therapeutic work
- Building tools to identify, process and regulate negative emotions and maladaptive behaviours
- Therapeutic driven environment to ensure consistency for the child
- Reflection and Resilience as core components of Therapeutic goals
- Promotion and development of life skills, resilience, education and vocations
- Future collaborative planning within care plan reviews
- Working closely with local services to promote step-down facilities / reducing package of care.
