Care Pathway

An inclusive step-down placement pathway.

  • Tier 4 CAMHS or inpatient mental health settings
  • Secure or welfare secure environments
  • Hospital failed discharges
  • Unregulated or emergency placements where stability has not been achieved

Our role is to provide a regulated, therapeutic bridge — offering containment, clinical oversight and consistency, while actively working towards reduced restriction and increased independence.

Step-down Placement Pathway

Each placement is clinically formulated, outcome-focused and designed with a clear trajectory towards step-down from the outset.

Through our recognised targeted model of care, we aim to step down the care each young person receives, ultimately leading to the effective transition into independent or semi-independent living.
