Our vision is simple
To provide truly specialist care within the community, reducing the needs for any child to require admission to hospital or return to inpatient settings by safely managing and mitigating risk.
We are Oakfield

About us
Oakfield’s ethos is strongly goaldirected, working in partnership with each person to promote and establish independence, resilience and social inclusivity.
Through our recognised targeted model of care, Red Amber Green step-way plan, we aim to review to step down the care each young person receives every 6-12 months, ultimately leading to the effective transition into independent or semi-independent
living and into adults services.
Our vision is simple
Through working within the NHS for nearly 20 years, Oakfield have become increasingly aware that some of our most vulnerable young people through not receiving the specialist support they needed in their community, are spending much of their lives within secure / inpatient settings.
Unfortunately, many of these young people were remaining in secure settings simply due to a lack of suitable provisions available within the community.
Oakfield recognised that it’s model of care and specialised therapeutic input within a hospital setting, if applied effectively within a community setting could be instrumental in the advancement of young people’s lives.
Having already the expertise, infrastructure and proven model of care in place, Oakfield have been uniquely positioned to provide an effective, meaningful service to this population, impacting and transforming lives for the better.
